Opt In With Web Form???

I have the trial version of Response Mailer Pro

I get how to set up the software for Optin for Addin Prospects but I don't get how to work it so that the web form data fields translate to the Actions fileds in software.

I have tried and tried to create a web form with the form fields named the same as the action inserts in the software but nothing works.

HOW EXACTLY is the web form coded???????

I have tried adding an extractor but the option to extract from the message body in the "extract email data" interface is greyed out for the "email data soruce" drop down and not an option.
